The City of Long Beach Vital Records Program maintains birth and death records for events occurring in Long Beach. Birth and Death Certificate requests can be made online, by mail or in-person for events that occurred within the past five years at the Long Beach Department of Health and Human Services.

- Death certificates for deaths since 2005

Monday - Friday from 8 a.m. to 4 p.m. (Closed from 12-1pm for lunch)

For individuals who were born or died outside of Long Beach/Los Angeles County, please call the county of birth or death directly, as each jurisdiction has its own procedures. Visit the California Department of Health website for a list of county registrars and recorders.

- You will be required to upload a valid form of identification for your order to be processed
- You must answer the Vital Verify Questionnaire at the end of the application to validate your identity when requesting the delivery option.
*An additional non-refundable $0.75 Vital Verify fee will be applied to all delivery orders
