- Application For Release of Traffic Collision And/Or Crime Report / Incident Information
This document should be used to request copies of traffic collision reports for insurance companies or crime reports/incidents investigated by the Long Beach Police Department. The Long Beach Police Department charges a non-refundable $29 fee for processing each application for release of a police report. This fee will be charged to process all types of reports: incident reports, arrest reports, traffic collision reports, and information-only reports. Victims of crimes will be exempt from the report request fee. - Additional Property Loss Form

How do I check the status of a police report request?

How Do I File a Police Report?
Filing a false police report is a crime. Under Penal Code Section 148.5 (A)To file a report using out Online Police Reporting systems please visit Online Police Reporting System. Once submitted, you will receive a temporary report number. After review, you will either receive an official police report number or rejection notice.
If your incident does not meet the criteria for online reporting, please call (562)435-6711 for Non-Emergency Police Response. Supplemental reports may be filed with our front desk at (562)570-7260 or in-person.

- Request a Police Report
- The Long Beach Police Department charges a non-refundable $29 fee for processing each application for release of a police report. Victims of crimes will be exempt from the report request fee.
- Additionally, a full set of fingerprints on a standard fingerprint card completed by local Police or Sheriff’s agency, (or an authorized fingerprinting facility), containing all personal data, including mailing address and zip code is required to request an arrest report.
- Incomplete requests will be returned.

What Are the Additional Requirements for Law Offices?
- The application must be signed by the Law Office representative.
- Attorneys representing parties involved must submit a signed authorization from the individual they represent.
- Client names should be legible, and signatures must be physical or DocuSign and cannot be typed out.
- Law Offices requesting a copy of an arrest report must also submit ink fingerprints of their client if listed as the arrestee in the report.
- All documents must be submitted along with a non-refundable processing fee of $29.

What are the Additional Requirements for Insurance Companies?
- Insurance companies must include a policy or claim number on the application form.

How do I Obtain Disposition of Arrest?
To obtain a disposition of your arrest (after appearing before a judge), please contact Los
Angeles County, Governor George Deukmejian Courthouse, Long Beach | Judicial
Branch of California or call (562)256-2314.
In addition, you can contact the California Department of Justice to obtain your own
criminal history printout that will contain disposition information (if available). Please
visit www.oag.ca.gov for more information. -
How do I Request a Clearance Letter?
A clearance letter, or “Letter of Good Conduct”, can be requested in person at the
address listed above. These letters are only for past or current residents of Long Beach.
Foreign passports are accepted if there is an official customs entry stamp and visas must
not be expired.
Be sure to include a copy of a valid government issued identification and a check or money
order for $41.00 made payable to the Long Beach Police Department.
For a notarized letter, an additional $15.00 cash is due at the time of pick up. -
What Forms of Payment are accepted?
If mailing in a request, only checks or money orders are accepted. If submitting a
request in-person, cash, checks, money orders, debit/credit cards (except Discover and
American Express) are accepted. -
How Can I Request a Records Sealing?
To petition the Long Beach Police Dept., the arrest must be an LBPD arrest and no court
case number or subsequent action from the court must have taken place. All responses
will be mailed to the address provided on the petition. Each arrest must have its own

How Can I Obtain a Declaration Letter?
A declaration letter can be provided when an arrest not pertaining to that individual is
showing up as theirs on a background, DMV record, etc... The individual must submit a 10-
print fingerprint card, a valid copy of their identification, contact information and any
information they have about the arrest that is being contested. This can be dropped off in
person or mailed in to LPBD. -
Where do I Submit a U-Visa Certification?
If the request for a copy of a crime report is in relation to a U Visa certification request,
please write on the outside of the envelope “U Visa Request”, include a self-addressed
envelope (postage not necessary) and submit to mailing address is listed above by mail or
in-person. -
How Do I Find Out If I Have a Warrant?
To check if you have any warrants, please call the Warrant Detail (562)570-5717,
Monday-Thursday 7:00am-1:00pm. -
How Can I Check on My Vehicles?
For status regarding a stolen, recovered, towed/impounded vehicle, please call Auto
Records at (562)570-7475. For copies of those reports, please follow the instructions listed

How do I request a report as a Government Agency?
AGENCY REPORT REQUEST REQUIREMENTS:
- Submit a formal request using your official agency letterhead.
- The Name of the Requester must be provided for audit purposes.
- Provide connecting information to the report you are requesting—Subject Name/DOB, Incident Type, Incident Date/Location, Vehicle Plate #, etc. (This includes requests for Calls for Service).
- Provide a reason for your request.
- Fees do not apply to agency requests.
BACKGROUND CHECK REQUIREMENTS:
- Submit a formal request using your official agency letterhead.
- Attach a notarized copy of your Applicant’s waiver.
- If you are a 3rd party, include authorization from the originating agency (must be on their official letterhead).
- Fees do not apply to background check requests.
- Provide a return mailing address. All responses are mailed.
