Please bring the completed and signed permit documents below to your appointment:

• FORM-005 California Licensed Contractors Declaration and Owner-Builder's Declaration
• FORM-006 Owner/Builder/Contractor Permit Proxy (if applicable)

In-person appointments are available Monday through Friday, from 8 a.m. to 3:30 p.m., and on Wednesday from 9 a.m. to 3:30 p.m., excluding holidays. Please note: we are limiting the number of appointments per customer to one per day. For each appointment, a limit of up to two projects may be processed based on the service.

When scheduling your in-person appointment, please include the project number and address in the “Notes” field in Step 3: Contact Details.
