Mobile Vaccination Team 

Mobile vaccination is a free service offered on weekdays to only Long Beach residents who are not able to travel to one of the City’s COVID-19 vaccination sites, such as persons with disabilities, those in long-term care, skilled nursing, elder care, sober living and other facilities as well as individuals who are not able to leave their home. 

Vaccination Request

Those who are able to access the COVID -19 vaccine through their primary care provider or at an ongoing vaccination site should continue to do so. 

  • To request a mobile vaccine for persons with disabilities or those who are homebound, please complete this form.

  • To request a mobile vaccine for long-term care facilities, older adult housing communities, and other community partners, please complete this form.

Staff will call or email you to confirm the details of your request and any additional information required. For further assistance, please call  the COVID-19 Information Line at (562) 570-4636 (open Monday through Friday, 9 a.m. to 5 p.m.)

Personnel and Equipment

Each vehicle is self-contained to administer up to 500 vaccinations and is equipped with supplies, vaccine storage units, tables, chairs and hand-washing stations to maximize the team’s time in the community.
