To request a certificate from the Eighth District Office, please submit the following Certificate Request Form. Please allow our office 7 business days to complete your request and coordinate a pick up time.

Presented at event: We cannot guarantee that Councilwoman Thrash-Ntuk will be available to present a requested certificate. A member of staff may attend on her behalf, or the certificate may be presented by the event organizers. To request Councilwoman Thrash-Ntuk’s attendance, please fill out the Event Attendance Request Form.  Please provide the date, time, location, and time of arrival of your event so our team can plan accordingly. Please allow for at least 5 business days from the date of your request.

Pickups can be made on Monday, Wednesday, Thursday, and Friday between 10am-5pm. Please allow for 2-3 business days from the date of your request.
