| Name(s) of business officer(s) partners |
| Tax identification number |
| Business and residential telephone number for one of the partners |
| Residential address for one of the partners. |
Fees and Deposits for Businesses
Fees and deposits vary depending on the type of business and the amount of gas and water usage anticipated. These amounts will be made available at the time of the turn on request.
Billing Cycle and Late Fees
Bills are prepared and mailed to the customer each month. The bill becomes past due if not paid within 15 days of the the “Bill Prepared” date. After 30 days a past due “Balance Forward” bill is produced. A “Shut Off ” notice is mailed 14 days subsequent to advise that if payment is not made in seven days the service may be terminated. A late fee will be charged if payment is not received prior to the late payment date printed on your bill. A late fee of $6.00 will be assessed on balances of $100 or less. A late fee of 6% will be assessed on balances of over $100.
Payment locations
Payments may be made in person at the Commercial Services Bureau located at 333 W. Ocean Boulevard, Long Beach City Hall, in the lobby by cash or check and, at this location only, by Visa, Master Card or ATM card. Payments may also be made at authorized payment locations. You also have the option to pay your bill on line at /commercial/utility/default.asp
Discontinuation of Service
Allow three working days to discontinue service.
Provide forwarding or mailing address.
Utility Users Tax: The current utility users tax in Long Beach is 5%, effective October 1, 2004.
