Anthem Blue Cross Premier HMO Plan
When you enroll in the Anthem Blue Cross HMO plan, you agree to use only Anthem Blue Cross doctors, facilities and medical groups for all of your medical care. You must choose a Participating Medical Group (PMG), or independent practice association (IPA), and Primary Care Physician (PCP) to manage your care. Anthem Blue Cross covers most services at 100%, with no deductible, as long as you use providers who belong to your PMG. Office visit co-payments are $20, and there are no claim forms.
Emergency room services require a $50 co-pay per visit. This co-pay is waived if you are admitted to the hospital.
* Note: Any care you receive without approval from your PCP is not covered.
Prescription drugs under the Anthem Blue Cross HMO Plan are administered through Express Scripts.
