A pharmacy licensed with the California State Board of Pharmacy and located within the City of Long Beach may register with the City of Long Beach Department of Health and Human Services (LBDHHS). Pharmacy Registration Form.
General Requirements
The legislation specifies certain requirements that participating pharmacies must agree to. Pharmacies are required to:
- Register with the City of Long Beach Department of Health and Human Services
- Certify that they will provide the consumer with verbal counseling or written information on all of the following:
- How to access drug treatment;
- How to access testing and treatment for HIV and HCV;
- How to safely dispose of sharps waste;
- Store hypodermic needles and syringes so that they are available only to authorized personnel; and
- Provide one or more of the following syringe disposal options:
- Provide an on-site hypodermic needle and syringe collection and disposal program;
- Furnish, or make available for purchase, mail-back sharps disposal containers that meet state and federal standards;
- Furnish, or make available for purchase, personal sharps disposal containers.
Registration Process
- Interested pharmacies who wish to sell, furnish, or accept for disposal hypodermic needles and syringes must register with LBDHHS. The registration form must be completed and returned to the address provided below.
- LBDHHS must respond to your enrollment request to be formally registered with the program. Registration becomes effective upon receipt of confirmation from LBDHHS and will expire on December 31, 2018, or upon earlier notice of termination.
- Registration may occur at any time during the demonstration period.
- There is no fee to participate in the DPDP.
- Pharmacies must be located in the City of Long Beach.
- Pharmacies must have a current and valid California license issued by the California State Board of Pharmacy.
- Please note that submission of a registration form does not guarantee registration until the City of Long Beach Department of Health and Human Services provides written approval of enrollment.
