Long Beach Diabetes Collaborative

The mission of the Long Beach Diabetes Collaborative is to develop a coordinated system of care and support for the prevention and management of diabetes in Long Beach through a community-based collaborative effort.

The Long Beach Diabetes Collaborative (LBDC) is made up of representatives from local healthcare organizations and community-based agencies as well as community members who have come together to support a
community-based planning effort to strengthen the system of diabetes care and prevention in the city of Long Beach. Projects of the LBDC focus on the needs of high-risk populations who are disproportionately impacted by diabetes and its associated health risks.

The LBDC is funded by a grant from The California Endowment. The grant was awarded to carry out four major goals: 1) establish a collaborative group to address the growing issue of Type 2 diabetes; 2) complete an assets/gaps analysis of the system of diabetes care for low-income adults, and identify systems strengths/weaknesses; 3) identify and pilot select best practices for reaching targeted at-risk populations; and 4) develop a collaborative action plan based on community priorities to create a higher functioning system of diabetes care. These goals are being met through the convening and on-going work of the LBDC which began meeting formally in September 2006.

Together, the Collaborative members are guiding a diabetes needs assessment process, which includes community dialogue meetings, surveys of targeted high-risk populations, and the inventory of local diabetes resources. The Collaborative is also pilot-testing several programs including a patient support model in a clinic setting and diabetes education and support activities. LBDC members are creating an Action Plan based on the information and insight gathered from the needs assessment and pilot activities.

Diabetes Care and Prevention Resource Guide
The Long Beach Diabetes Collaborative is pleased to bring you this guide of free and low-cost diabetes care and prevention resources for the greater Long Beach area. The Collaborative hopes that this guide will increase awareness of the services that exist locally, and will help those with or at-risk for diabetes find the care, support and services needed to live healthy, active lives.

LBDC Meeting Dates
Long Beach Diabetes Collaborative meetings are held quarterly at the Miller Family Health Education Center, 3820 Cherry Ave.  Next meeting: May 12, 2009 - Time TBD
