Click here for our "Fax Referral Form" | | The Medi-Cal & Healthy Families Outreach (MCO) Program works to reduce the number of uninsured children and improve the overall health of Long Beach by providing access to health care. Together with area CBOs, health care providers and schools the MCO Program provides latest information on the available resources. MCO Program also assists in enrolling children in Medi-Cal, Healthy Families, Healthy Kids and Kaiser Child Health Plan. The MCO Program works together with our CHDP Administration by promoting Child Health and Disability Prevention (CHDP) Program as a gateway towards increasing access to health care for the uninsured. The Medi-Cal Outreach Program provides the following services in English, Spanish and Khmer: | | Outreach and Education: MCO Staff participates in monthly events throughout the City of Long Beach promoting the available health programs. Methods of outreach include: information table/booth, presentations and school events. | | | Verify Eligibility: MCO Staff screens for eligibility in person or by phone in about 5 minutes. | | Referral Services: MCO Staff provides information of other human services resources specific to the needs of the client. | | Application Assistance: MCO Staff are trained and State Certified to provide application assistance to qualifying families. | | | Health Insurance Programs Medi-Cal for Children & Pregnant Women Healthy Families Healthy Kids (Ages 0-5) Access for Infants & Mothers (AIM) Kaiser Child Health Plan (Currently Closed), call to be placed on an interest list | | Case Management: Applicants are contacted within 90 days of applying to assure a successful enrollment. At the 6th month clients are contacted to assist families in navigating the system and assist them in using the benefits. And at the 11th month clients are contacted to help them in keeping their benefits by providing renewal assistance. | | Troubleshooting: MCO staff provides assistance in resolving issues with Medi-Cal and Healthy Families. | | | Walk-ins and appointments are welcomed Monday through Friday from 8:00-5:00 PM.
