Certificate of Recognition, Character Reference or Letter of Congratulations
Requests should be made at least 10 business days in advance of the date the document is needed. Requests should be made using this form and emailed to firstname.lastname@example.org. The Vice Mayor’s Office may modify, edit, amend or deny any Reference Letter or Certificate of Recognition request. All completed items should be picked up within 14 days of notice. Please specify in the application should you need them sent via mail.
